I acknowledge that it is essential for nurses to use the Malnutrition Universal Screening Tool to assess Sita s nutritional status, which aids in minimizing the risk of malnutrition and dehydration (Fisher and Scott, 2013). Sita was referred to dieticians who advised nurses to give healthy foods and high-calorie drinks to her. I always encouraged Sita to eat the recommended foods and drink more fluids. I monitored her food and fluid intake regularly and documented it in the food and fluid chart. Further, I and my mentor did Sita s pain assessment using the pain assessment chart and administer pain relief drugs as prescribed by the doctor. Older people are more likely to catch infections because of their weak immune system (Pignolo et al. , 2011). Sita was at increased risk of developing urinary tract infection triggered by the catheter bag inserted in her. Accordingly, I and my mentor carried out an appropriate risk assessment in Sita to reduce the risk of getting urinary tract infe ction. Whilst, inserting a catheter bag in Sita, we followed the aseptic non-touch technique procedure and washed our hands carefully before and after coming in contact with her to minimize cross-infection.
